Students should read the rules and instructions for all forms that may be applicable to your project; these forms should be filled out, signed and copied. All signed originals should be kept by the students. Send copies of all paperwork to:

1) All students must submit completed:
- Form 1 – Checklist for Adult Sponsor
- Form 1A – Student Checklist
- Form 1B – Approval Form and a Research Plan
2) Additionally, students with projects meeting the following criteria, complete forms:
- Form 1C – required for studies done in regulated research institution (industrial or university laboratory; not home or school)
- Form 2 – Qualified Scientist Form – required for all studies involving human subjects, vertebrate animals, BSL-2 potentially hazardous biological agents and DEA controlled substances
- Form 3 – required for hazardous chemicals, activities or devices
- Form 4 – required for studies involving human subjects
- Form 5A – required for studies involving vertebrate animals conducted in regulated research institution
- Form 5B – required for studies involving vertebrate animals conducted in non-regulated research facility
- Form 6A – required for projects involving microorganisms, rDNA, and fresh tissue, blood or blood products;
- Form 6B – required for projects involving fresh tissue, primary cell cultures, blood, blood products and bodily fluids
- Form 7 – required which are continuations of previous years’ work
- Only projects that have sent in all paperwork by registration deadlines
will be eligible to participate.
